Confirm the transfer plan
Ask the treating team about the destination, preferred timing, transport requirements, and contact person. Confirm whether the transfer is urgent, scheduled, or part of a discharge plan.
Keep essential information organised
Carry identification, contact details, medication information, prescriptions, appointment details, and documents requested by the hospital. Keep sensitive records secure and do not upload them to an ordinary public website form.
Explain mobility and comfort needs
Tell the ambulance coordinator whether the patient needs a stretcher, wheelchair, assistance with stairs, an attendant, or a particular vehicle arrangement. Share the full pickup and destination addresses.
Prepare the attendant
One family member should remain available for calls and updates. Carry water, basic personal items, chargers, and any other items advised by the hospital. Avoid carrying unnecessary valuables.
Ask about arrival arrangements
Confirm the receiving entrance, hospital contact, expected arrival process, and whether the ambulance team will provide updates during the journey. Admission decisions remain with the receiving hospital.
