What you will be asked for
The enrolment form collects operational details only. It never asks for patient records, diagnoses or insurance information.
- Owner or company name and a contact person
- Email, phone, city, state and current operating areas
- Fleet size, ambulance types and basic equipment carried
- Driver and paramedic availability, and operating hours
- Registration or licence details, where applicable
What happens after you submit
You receive a reference number on screen. The TEN team reviews the details and contacts you using the information provided. Nothing about your operation is published without your agreement.
